Our mission is to build a premier Geriatric Care Outreach Program, which provides elderly and disabled individuals with a personalized evaluation of their needs to ensure quality affordable care in a variety of settings, allowing the person to age in place with dignity and comfort.

Vision Statement: Tri-State Care Alternatives is a Geriatric Care Management Information and Referral Company designed to assist older adults and their families through the maze of health care services that are available to them in their respective place of residency.
Aging processes vary greatly within our society and while some older adults continue to have excellent health and independent functioning warranting an approach focused on health maintenance and future care planning, other elderly may present with a variety of medical conditions, and become dependent on their families and health care professionals to meet their needs in daily activities, coordination of medications, and a palliative approach.
Target Population: Serving older adults in the Tri-State territory, who are in need of Care Coordination and Care Management due to a variety of health conditions that may include:

Alzheimer’s disease/related dementia disorders, including but not limited to: Parkinson;s disease, Vascular dementia, Lewy Body Dementia and many others

Referrals: Referrals are accepted from family members, health care professionals, hospitals, nursing homes, elder service agencies, visiting nurses, friends and neighbors.

Tri-State Care Alternatives provides a quality alternative to institutional care or other more expensive services with affordable live-in care assistants.
In addition to national background and criminal checks, care assistants are trained to address a variety of eldercare issues and a matched with Care Recipients to ensure an individualized personality match.
